JOB SUMMARY
Responsible for client placement services, case management and coordination. Support clients and assist in developing vocational skills, obtaining goals and assist in obtaining competitive employment. Must be able to represent the organization to other stakeholders including employees, families/guardians, outside agencies, and funding sources. Ensure activities and operations are carried out in compliance with all regulatory agencies and with Winona ORC’s mission, vision, goals, and policies.
CORE ACCOUNTABILITIES
- CASE MANAGEMENT (30%) – Develop Individual Service Plan (ISP) for each assigned client. Proper implementation and understanding of the intake/discharge process and procedures. Assist in obtaining competitive employment. Train and support (job coach) clients at the workplace. Advocate for clients as necessary to accomplish employment related plans and goals. Measure and evaluate retention at 90 days. Follow-up with EE clients twice per month and once with Employer. Work with outside agencies to coordinate services and to be an active member of the client’s support team. Refer clients to other outside agencies for support services as needed. Ensure that programs are in compliance with all governing bodies including federal, state and county rules and regulations, DHS-245D, CARF and DOL. Provide programming that meets the needs and goals of clients using person center planning approach.
- EMPLOYMENT DEVELOPMENT 20%) – Initiate, develop, and implement employment and other programming for the clients as requested. Work with clients to expand skills and training including but not limited to, independent living skills, vocational issues and social interactions. Provide individual support to clients in the areas of vocational counseling, job training, social and independent living skills. Advocate for clients as necessary to accomplish their vocational goals.
- TEAM PARTICIPATION (15%) – Attend all assigned internal meetings. Attend annual client team meetings and other necessary meetings. Participate in all activities that are requested, and perform other duties as assigned. Provide honest feedback in meetings. Support team decisions. Seek out supervisor with concerns.
- COMMUNICATION (15%) – Effectively communicate in a timely manner with staff, clients, families/guardians, outside agencies, and funding sources. Communicate in a manner that protects and ensures client confidentiality.
- ADMINISTRATION (20%) – Accurate and timely submission of client timecards. Maintain and update clients’ employment plans including risk management plans. Maintain documentation which includes writing annuals and semi-annuals, monthly progress notes and contact with clients and their employers. Maintain Purposeful Outcome Planning data for assigned rehabilitation programs. Assist with preparing outcome data reports. Comply with requirements of regulatory agencies including policies on 245D responding to and reporting incidents and following all safety practices and Winona ORC’s mission, vision, goals, and policies.
MEASURES OF SUCCESS/STANDARDS
- CASE MANAGEMENT – Individual Service Plans (ISP) are developed for each client. Potential clients are screened for placement services. The intake and discharge procedures are understood and followed. Client referrals and service coordination are implemented in a complete and timely manner.
- EMPLOYMENT DEVELOPMENT Work with clients to expand skills and training including but not limited to, independent living skills, vocational issues and social interactions. Clients are referred and placed in competitive employment and other services in the community. Advocate for clients as necessary to accomplish their vocational goals. Clients are coached at the workplace as needed. Client accomplishes employment goals and plans. The client’s job retention is measured and evaluated at 90 days. EE client follow up twice per month with client face to face and once with Employer per month.
- TEAM PARTICIPATION – Attendance and participation requirements at all assigned meetings, activities, and other necessary meetings are met. Team decisions are supported and concerns have been addressed. All duties assigned by supervisor have been completed.
- COMMUNICATION – Communication with staff, clients, families/guardians, outside agencies, and funding sources is maintained in an effective and timely manner. Client confidentiality is maintained.
- ADMINISTRATION– Documentation is maintained and updated. Outcome data reports are completed. Complies with requirements of regulatory agencies and Winona ORC’s mission, vision, goals, and policies.
KNOWLEDGE, SKILLS, & ABILITIES
Education: | · Bachelor’s degree in Human Services or related fields. |
Experience: | · At least two to three years experience in related field
· Ability to provide support services to people with disabilities in their employment setting · Computer literate. Proficient in MS Office |
Language Skills: | · Proficient writing, speaking, and communication skills in English |

Wage starts at $17.50/hour and may be increased after successful completion of 90 days, following a performance review.
